3.  After Death Wishes

NOTE: This page shows Part 3 of a typical Advance Directive. See Part 1 (Appointment of Health Care Agent or Proxy) and Part 2 (Living Will).

After death wishes is additional documentation that many include in their Advance Directive to let medical staff and caregivers know exactly what you would like to do with your remains. This document usually includes whether or not you would like to donate your organs and which ones, if you would like to donate your body to science, and what your funeral arrangements will be. You can also add specific treatment and handling requirements according to your religious or spiritual beliefs.
